Notes for the Real-name System of Patient ID Cards and Registration

In order to protect patients’ rights and maintain medical order, the Real-name System of Patient ID Card and Registration is necessary. More information is listed as follow:

1. Please choose the following ways to provide patient’s basic information for Patient ID Card Registration:

1) Show patient's identity card

2) Show patient's valid identity certificates, such as University ID Card, Residence Booklet or Driver's License.

3) Fill in a basic information form (including name, sex, and date of birth or age etc.).

4) Patients with medical insurance in Haining, Hangzhou or any other cities in Zhejiang need to show both Health Insurance Booklet and Social Security Card.

2. Register with Patient ID Card, a valid identity certificate sometime is necessary. No registration withdrawal or name changing without doctors’ approval.

3. Please keep your Patient ID Card and take it when you go to campus hospital because of the real-name system. The latest registered Patient ID Card would be valid if a patient owns more than one.

4. It is not allowed to register with other person’s Patient ID card. Hospital has the right to reject to engage the patient whose basic information is false and all the consequences caused by false information would be born by the guardian, the patients themselves or their families.
